As a New Patient - Preparing for your first visit
Understanding YOUR Insurance Policy

Each insurance policy is different, and the amount of coverage for procedures depends on your specific health plan, so it is important to read, know and understand YOUR policy.

Some insurance companies require authorization before patients can receive treatment, while others do not.  

Since patients are fully responsible for all bills related to treatment, regardless of what their insurance policy covers, it is important for you to know these details to minimize your out-of-pocket expenses.

To find out the specifics of your policy, contact your insurance company (the phone number is usually on your insurance card). They should be able to tell you about in-network and or out-of-network coverage for care with Donna Reid Washington, MHS, LPC , as well as what your financial responsibility will be if you become a patient here.

We have developed a form for you to use when calling your insurance company, this form will tell  you what questions you should ask.  
Carefirst BCBS HMO, PPO and FEP Plans
Tricare North, we will require the Name of your PCP - Primary Care Physician/Doctor and an authorization from Tricare maybe also be required
Aetna, HMO, PPO, POS, Open Access and Choice Plus Plans

If payment is necessary on your first visit, CASH payment is required.  You will be able to use other forms of payment after the first visit.
